ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM
I hereby accept all responsibility for the inherent risks associated with my participation in any and all activities related to this event. These risks may include, but are not limited to, those arising from negligence or carelessness on the part of individuals or entities being released, as well as risks associated with the use of dangerous or defective equipment or property owned, maintained, or controlled by them. I acknowledge the potential liability of these individuals or entities without fault on their part.
I confirm my physical fitness and readiness for this activity, having prepared or trained sufficiently for my participation. I have not received advice from a qualified medical professional advising me not to participate, and there are no health-related reasons or issues that would prevent my involvement.
If I am less than 14 years old, I understand that I must have the consent of my parents or guardians to participate in any registered event or class.
I understand that this Accident Waiver and Release of Liability Form will be used by the organizers, sponsors, and hosts of the activity in which I am participating. It will govern my actions and responsibilities during this event.
In consideration of my participation, I, for myself, my heirs, executors, administrators, successors, and assigns, take the following actions:
(A) I WAIVE, RELEASE, AND DISCHARGE any and all liability, including but not limited to liability arising from negligence or fault, of the individuals or entities being released, for my personal injury, disability, property damage, theft, or actions of any kind, including incidents during my travel to and from this activity. These entities or individuals include At The Park, their organizers, trainers, volunteers, representatives, and agents, as well as the activity holders and sponsors.
(B) I INDEMNIFY, HOLD HARMLESS, AND AGREE NOT TO SUE the entities or individuals mentioned in the previous paragraph for any and all liabilities or claims arising from my participation in this activity, whether due to their negligence or otherwise. I acknowledge that International Day of Yoga Dubai, its organizers, trainers, volunteers, representatives, and agents are not responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf.
I am aware that this activity may challenge my physical and mental limits and may entail risks such as death, serious injury, and property loss. These risks may arise from factors including terrain, facilities, temperature, weather, participant condition, equipment, vehicular traffic, hydration levels, and actions of other individuals, including participants, volunteers, monitors, and activity producers.
I consent to receive medical treatment that may be deemed necessary in the event of injury, accident, or illness during this activity. While participating, I understand that I may be photographed, and I grant permission for my photo, video, or film likeness to be used for legitimate purposes by the activity holders, producers, sponsors, organizers, and assigns.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY, AND BY REGISTERING FOR THIS EVENT, I AM DOING SO OF MY OWN FREE WILL.
